The form provided is a substitute W-9 used by the State of South Dakota, which&nbsp;will allow for you or your agency to receive reimbursement for services delivered. Complete the banking information section to ensure direct deposit. This document will be required to complete the enrollment form in Step 3.<\/span><\/li><li><span style=\"font-size: 12pt;\">Current Certificates of Insurance: Insurance documents are referenced in the sample <a href=\"\/wp-content\/uploads\/Agreement-Template_through-2022.pdf\">Agreement Template: Number 22 Insurance<\/a>. These certificates of insurance will be required to become an approved provider in the Behavior Health Voucher Program.
